- The online pre-registration form is used ONLY for BIDH Dental Hospital Thailand.
- Please use this form only if
- you are a new patient and ths is you’re first appointment to BIDH
- you have a confirmed appointment booking by phone or email from our staff
- If you do not have an appointment or are unsure if you have a confirmed appointment, please kindly contact us
If you are a returning patient to BIDH, there is no need for any pre-registration as you’re information is already logged within our system. Please do however inform our staff should there be any updates on you’re contact and medical information.
